Physical Forms

In order to participate

PHYSICALS

  • ALL   Athletes and Cheer 7th-12th Grade are required to have a NEW ANNUAL PHSYCAL
  • ALL Band, Dance, and JROTC 9th-12th are required to have a NEW ANNUAL PHSYCAL
  • Please follow the instructions below how to access your physical form

 

  1. You will need the parent’s Parent Portal log – in information.
  2. Please fill out all of the Medical History Questions <SUBMIT>

Don’t forget you email address where it asks for it.

    1. Electronically sign all off the UIL Forms. <SUBMIT>
    2. A new screen will open. If the PDF for your physical does not pop up. Click on the link to pull down load it.
    3. There should be menu items to print the physical.
    4. If there are no menu items to print the physical [File] scroll over the bottom center section of the PDF and a menu should show up with the PRINTER symbol. Click it and Print it.
    5. Take the Physical form to your physician.
    6. Turn in physical to Mr. Wheeler- Athletic Trainer or Coach McEachern prior to July 31st if participating in Band, Cross Country, Dance, Football or Volleyball
  • Turn in physical to Mr. Wheeler- Athletic Trainer or Coach McEachern no later than registration August 15th for all other sports and JROTC.

ECGs CONSENT FORM CLICK HERE

  • ALL 7th Grade Athletes are required to have an ECG.
  • ALL 9th Grade Athletes, Band, Cheer, Dance, and JROTC are required to have an ECG.
  • Any 8, 10-12th Grade student who does NOT have a CURRENT ECG (from 7th or 9th Grade) and will be participating in any of these programs who did
  • ANYONE NEW to the district in 7th -12th Grades who will be participating in the above activities are required to have an

 

You do NOT need any forms to take to your physician for the ECG. We do need a report from he the doctor stating that you had and ECG and you PASSED the ECG.

If your physician’s office does not perform ECGs please contact Mr. Wheeler-Athletic Trainer or Nurse Holly. We can help you locate a provider.

 

Turn in ECG to Mr. Wheeler- Athletic Trainer or Coach McEachern prior to July 31st if participating in Band, Cross Country, Dance, Football or Volleyball
