WHO NEEDS A PHYSICAL FOR 2020-2021?

 

This policy for physicals is set by the UIL.

  • If your student participated in an extra-curricular activity that requires a physical in 2019-2020, and turned in that physical—they DO NOT NEED A NEW PHYSICAL for 2020-2021.  
You are required to fill out a NEW Medical History form.  If you can answer YES to any of the first six (6) questions, then your student will need a NEW PHYSICAL. 
 
  • If your student DID NOT participate in an extra-curricular activity that requires a physical in 2019-2020, they DO NEED A NEW PHYSICAL for 2020-2021.
 
  • If your student is NEW to the district. They will follow the policy stated above. You MUST turn in a copy of the 2019-2020 PHYSICAL to the Head Athletic Trainer.

 

OFFICIAL UIL STATEMENT:

For the 2020-21 school year only, any participant in athletics or marching band, in any grade level, who has not previously completed a preparticipation physical examination (PPE), and been cleared for participation, will be required to complete the medical history form as noted in current rule, as well as a PPE prior to participation in any UIL practices, games, performances, or matches.

 

As required under current rule, all students participating in UIL marching band and/or UIL athletic activities will be required to complete the UIL Medical History Form. Any “yes” answer to any of questions 1-6 on the Medical History Form will require a student to receive further medical evaluation, which may include a physical exam, and receive written clearance from a physician, physician assistant, chiropractor, or nurse practitioner before any participation in UIL practices, games, performances, or matches. Only those students with “yes” answers to those questions are required to receive written clearance through further medical evaluation.

Please note that this is only for the 2020-21 school year, and local school district policies still apply. Schools may still require physical examinations of students before allowing participation in these activities, and such decisions are under the jurisdiction of each local school district

 

Which extra-curricular activites need a physical?

  • High School- all athletes, band, cheer, dance and JROTC
  • Middle School- all athletes, cheer, and any 8th-grade band members who will be marching with the HS.

WHO NEEDS AN ECG FOR 2020-2021?

  • If your student participated in an extra-curricular activity that requires an in 2019-2020 and turned in that ECG—they DO NOT NEED A NEW ECG for 2020-2021.
 
NOTE: The 2019-2020 8th Grade students who have an ECG will carry over to 2020-2021.    We will require a NEW ECG for the 2021-2022 school year.
 
 
  • If your student DID NOT participate in an extra-curricular activity that requires an ECG in 2019-2020, they DO NEED AN ECG for 2020-2021.
 
  • If your student is NEW to the district. They will follow the policy stated above. You MUST get an ECG   and turn in a copy of it to the Head Athletic Trainer. 

 

 

Which extra-curricular activites need an ECG?

  • High School- Incoming 9th-grade students involved in athletes, band, cheer, dance and JROTC
  • Middle School- Incoming 7th-grade students involved in athletes and cheer
  • Also any student new to the district involved in the above activities.

 

When are the Physicals and ECGs this year?

 

UNFORTUNATELY, DUE TO COVID-19 virus we will NOT be hosting physicals or ECGs at the present time.   If things change, we will update you!

What is the difference between an ECG and an EKG?

 

The fact of the matter is that an ECG and an EKG are the exact same thing. That's right, the most surprising difference between an ECG and an EKG is that there is no difference at all. ... It's actually quite simple—when the word electrocardiogram is translated into the German language, it is spelled Elektro-kardiographie.

 

What forms do I need to take to family doctor for the ECG?

 

NO FORMS are needed.    We do need a report/note from the doctor stating that an ECG was completed and passed allowing the student to participate.

Where do I fill in the MEDICAL HISTORY form?

The Medical History form is accessible with the other required UIL Forms.   The link is located at the top of this page.

 

Where do I get a copy of the physical form for my doctor to sign?

1)  Make sure pop-up blockers are turned off or set to allow it for this site.

2) YOU WILL NEED YOUR Parent’s--- PARENT PORTAL LOG IN INFORMATION!

3) Follow these steps.

  • Access the District Website huffmanisd.net
  • Select-----------------Extra Curricular
  • Select----------------Athletics
  • Select----------------Physical Forms
  • Click on Link------ UIL ONLINE FORMS, Medical History & Physical Form CLICK HERE

3) Log in with your Parent’s Parent Portal email and password

Click on the Orange circle labeled Athletics Participation   

Check Box Demo

                                                It says optional (but it’s NOT) !!!!!!!

4) Answer all of the health questions

5) SUBMIT-   Electronically sign all of the forms

6) SUBMIT-    After you complete the form, the orange O changes to a green checkmark with a download button.

If you need a physical form to take to the doctor, you can now download it to your computer.

 

Why can’t I log into the Parent Portal?
